 "To guide individuals down their desired path while providing them with a variety of supports and services to aid in their journey"

Navigating the sea of services that are available to you and your loved one with a disability can be very challenging. A Service Coordinator can help guide you on this journey. The Arc of Livingston-Wyoming has a team of 16 Service Coordinators serving individuals in Livingston and Wyoming counties, some with nearly ten years of experience in the field.

Service Coordinators provide assistance to individuals and families in a variety of areas, with some of the most common activities being:

  • Obtaining and maintaining Medicaid
  • Pursuing Wavier enrollment (often times to secure Medicaid for children)
  • Assistance and advocacy at Committee of Special Education (CSE) meetings
  • Linkage to summer camps (securing funding if needed)
  • Linkage to recreation services, for both children and adults
  • Applying for and maintaining benefits, such as SSI
  • Applying for Family Support Services (Family Reimbursement, Special Sitters, etc.)
  • Applying for NY State Park Access Pass (allowing free admission to state parks)
  • Assistance in transition to adult services (i.e. sheltered workshop, supported work, day habilitation)
  • Linkage to at-home supports (i.e. residential habilitation, hourly respite)
  • Assistance and guidance in obtaining and maintaining housing (i.e. applying for Section 8)
  • Pursuing guardianship
  • Exploring and securing residential placement
  • Pursuing funding for adaptive equipment and environmental modifications
  • Assisting individuals in exploring their personal interests (i.e. photography, dance, reading, etc.)

With so many programs and services available, it is critically important to have a trained, knowledgeable person available to listen, answer questions, and be a guide to the right activities and support based on individual needs and desires.  An Arc of Livingston-Wyoming Service Coordinator can fill that role.  For intake, contact Cathy Sullivan at (585) 658-3311.
